Job Description

Job Summary:

Performs duties that consist of personal care and household tasks for patients/clients in their homes.

Level of Responsibility:

Works under the supervision of the Supervising Nurse and the Registered Nurse.

Duties and Responsibilities:

Performs duties as ordered by the physician (included in the plan of care) and as assigned by the Registered Nurse or skilled professional.

Performs duties that are permitted under state law and consistent with home health aide training.

Provides hands-on personal care, such as baths, oral hygiene, nail care, foot care, skin care, bowel/bladder care, pressure relief, dressing patient, and feeding patient.

Performs household tasks related to patient's medical needs such as bed linen changes, patient's laundry, light housekeeping, shopping, errands, etc.

Takes and records temperature, pulse, respiration, blood pressure, and weight according to state regulations.

Performs simple procedures as an extension of therapy or nursing services.

Assists with ambulation and exercises.

Assists with administering medications that are ordinarily self-administered.

Communicates any pertinent information about the patient/client’s condition, including any changes in the patient’s condition and ongoing needs to the registered nurse or other appropriate skilled professional in a timely fashion.

Documents all care and services provided according to professional standards and agency policy and procedures.

Adheres to all agency policies and procedures.

Completes all mandatory agency training and in-service activities.

Participates in the agency’s quality management activities.

Establishes and maintains effective channels of communication and working relationships.

Performs all aspects of patient care in an environment that optimizes patient safety

Maintains confidentiality in all aspects of job performance.

Performs related duties as required.

Minimum Qualifications:

Completion of a program approved by the State for Home Health Aide or Certified Nurse Aide training or equivalent.

Certified as a Certified Nurse Aide by the State of Colorado.

Previous experience in the home care industry preferred.

Knowledge, Skills, and Abilities:

Knowledge of principles and procedures of personal care and safety practices in the home care setting.

Ability to foster constructive and therapeutic relationships with patients/clients and their families.

Ability to communicate effectively, both orally and in writing.

Special Requirements:

Current, valid license to practice as a Certified Nurse Aide in the State of Colorado (or other compact state).

CPR certification.

Current, valid driver's license.

Access to a reliable automobile.

Exposure/Risk Category:

Tasks involve occupational exposure to blood, body fluids or tissue.
